You can now pre-register for a COVID-19 vaccine appointment below if you meet both of the following criteria:
- All special education workers who live or work in Peel or education staff who live or work in Peel hotspots (listed below)
- OR
- Adult with highest risk or high risk health condition and up to one essential caregiver
- *High priority health conditions are outlined by the Ministry of Health. Vaccinations will begin with highest-risk and high-risk conditions. The individual and essential caregiver do not have to book their vaccine appointments at the same time. For more information on eligible essential caregivers, visit page 15-17 of the Ministry of Health’s Guidance for Prioritization of Phase 2 Populations for COVID-19 Vaccination.
- OR
- Indigenous Adult 18 years and older at the time of pre-registry
- OR
- 60 years or older (born in 1961 or earlier) and are a resident of Peel or live within the Trillium Health Partners catchment area, including Mississauga or Etobicoke
- OR
- 50-59 years or older (born in 1971-1962) within select COVID-19 hotspots in our community.
- Provincially designated eligible hotspot postal codes:
- Peel: L4T, L4W, L4X, L4Z, L5A, L5B, L5R, L5C, L5K, L5L, L5M, L5N, L5V, L5W, L6P, L6R, L6S, L6T, L6V, L6W, L6X, L6Y, L6Z, L7A, L7C
- Etobicoke: M9A, M9B, M9C
If you meet any of the above criteria, the THP COVID-19 vaccine scheduling team will reach out to you to book an appointment based on available appointments.
Pre-register if you are adult (18+) with health conditions as
outlined by the Ministry of Health or are an approved essential caregiver and have a written attestation form from your physician. Your essential caregiver (if applicable) will also need a separate written attestation form from their physician. You or your caregiver will
not be given a vaccination if you do not each have a written attestation form from your physician.
